Insurance & fees
Clear pricing. In-network with select insurers.
I believe you should know what care will cost before you walk in the door (or sign on to telehealth). Here's how billing works in this practice.
In-network plans
I'm currently in-network with:
- Blue Cross Blue Shield (Regence, Premera, Anthem)
- Moda
- PacificSource
- UnitedHealthcare / Optum
- Providence Health Plan
Coverage details, copays, and deductibles are set by your plan. I'll verify benefits before your first visit, but you remain responsible for any charges your insurer doesn't cover.
Out of network
I do not accept:
- Medicare
- Medicaid and Oregon Health Plan (OHP)
If you have Medicare, Medicaid, or OHP as your primary insurance, I'm unable to see you as a patient in this practice. I'll do my best to help point you toward clinicians who can.
If I'm out of network with your commercial plan
I can provide a detailed superbill after each visit that you can submit to your insurer for possible out-of-network reimbursement. Your plan's OON benefits determine what (if anything) comes back to you. I'm happy to talk through what to expect at our consultation. Self-pay rates are shared directly during your inquiry so we can answer questions in context.
Cancellations
Please give at least 48 hours' notice to reschedule or cancel. Late cancellations and no-shows are billed at the full session rate; most insurers do not cover these charges.
Good faith estimate
Under the federal No Surprises Act, self-pay patients are entitled to a good faith estimate of expected charges. I'll provide one at or before your first visit.
