New patients

Request a consultation.

To make sure we're a good fit before scheduling, I ask all new patients to complete a brief intake form. I review every submission personally and reach out to schedule if there's availability and I believe I can genuinely help.

01

Complete the form

Fill out the short new patient intake form — it takes about five minutes. Share what brings you in and a bit of your history.

02

I review your request

I read every submission myself. I'm looking for patients I can genuinely serve well — not just filling a slot.

03

I'll be in touch

If there's availability and I think we're a good fit, I'll reach out within five business days to schedule your consultation.

Before reaching out, a few things to know

  • I practice telehealth only. All visits are conducted over video. You must be physically located in Washington or Oregon at the time of each appointment.
  • I keep my panel small. I don't maintain a waitlist — it only adds uncertainty and delays the care you need. If I have availability and we're a good fit, you'll hear from me within five business days.
  • Not sure if we're a fit? Review the Good Fit page first — it outlines exactly who I work best with and who I'll refer elsewhere.
