New Patients
I am currently accepting new patients throughout Washington and Oregon via telehealth!
- Choose “Initial Consultation” to request a 50-minute appointment as a new patient.
- Choose “Intro Call” if you would like a 15-minute phone call before requesting an appointment (best if you have specific questions before establishing care).
- You will receive an email with the required new patient paperwork. Please complete this at least two days before your appointment or your consultation will be cancelled.
Insurance
I am in-network with the following health insurance plans:
- Blue Cross Blue Shield (Regence, Anthem, Premera)
- Providence Health Plan
- Moda Health
- Pacific Source
- Optum/United Healthcare
- First Choice Health Network
I am not in-network with Medicare or Medicaid/OHP.
As a courtesy, I will check your insurance eligibility, but please note that you are ultimately responsible for determining your insurance coverage and out-of-pocket costs.
Cash Rates
If you do not have health insurance, or I do not accept your plan, please email admin@jasonjarriamd.com to inquire about cash rates. I may be able to provide a superbill to reduce your out-of-pocket cost.
